Lyra EAP - Wisconsin AND Florida Residents ONLY

I am a Lyra EAP provider. If you will be using your EAP benefits to cover services, please provide your date of birth, sponsoring company, and whether or not you are an employee or dependent of an employee in the message of my contact form. If you are a dependent please provide the eligible employee name and date of birth. Lyra EAP benefits will cover both individual and couples therapy.

Cancelation Policy:

Clients are required to provide 24 hours notice if they need to cancel and/or reschedule. Providing less than 24 hours notice or no showing an appointment results in responsibility for full session fee.

Fee will be charged on the day of the missed appointment.

Insurance:

To keep my practice focused on client care without limitations, I do not bill through insurance panels.

Individual Therapy:

I accept HSA/FSA as payment.

If you have health insurance, I recommend contacting your provider to confirm whether or not you have out of network mental health coverage. If you do, I can provide you with a receipt of services, also known as a superbill, that you can submit to your insurance provider for possible reimbursement of all, or a portion, of what you pay out of pocket while in therapy.

When contacting your insurance company, ask the following questions:

  1. Do I have out of network mental health benefits? If I do, is telehealth covered?

  2. If yes, what percentage of the cost do you reimburse?

  3. Do I have a deductible I need to pay before I get reimbursed? If yes, what is the deductible amount? Has my deductible been met?

  4. How many sessions per year are covered?

  5. Is a prior authorization from my Primary Physician required?

  6. How/Where can I submit a superbill?

Couples Therapy:

In order to provide a superbill, a diagnosis must be rendered. There is no diagnostic code to cover couples therapy, meaning you cannot be reimbursed for paying out of pocket.

Additional Options:

  • Check with your HSA/FSA to see if they will cover couples therapy

  • Talk with your tax preparer and ask if therapy expenses can be deducted from your taxes as an out-of-pocket health expense

Good Faith Estimate:

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

●    You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
●    Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
●    If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
●    Make sure to save a copy or picture of your Good Faith Estimate.

For more information on your right to a Good Faith Estimate visit https://www.cms.gov/nosurprises  or call 800-985-3059
